1. Click the “Download” link below. A window will open asking whether you want to open or save the file.
   
   
2. Click “Save”.
   
   
3. The next window will identify a location to save the file, usually “My Documents”. 
   
   It is recommended that you save it in “My Documents” or on the “Desktop” so that you will have easy access to it when you are ready to complete the forms.
